Our services
At Innovative Care Services, we provide services for individuals who have received a diagnosis or evaluation by their County Services or by Aging and Disability Resource Center (ADRC) for comprehensive community services, residential services or community-supported living services and are then referred to us by their Managed Care Organization (MCO) or county.
Upon receiving the referral, the team at Innovative Care Services joins the care team and immediately seeks to learn the dreams and desires of the people we support. That is at the core of how we then provide, adapt and develop our approaches and techniques to best meet the needs of each individual.
We encourage every client we serve to be actively involved in determining when and how their support is provided. We believe this team approach is critical to helping each client accomplish their goals and live the very best way possible.
Community-supported living services (support in your home)
Once you or your loved one has received an evaluation of capabilities and desires, Innovative Care Services develops a specific Individual Support Plan (ISP) with you.
From there, we work to ensure the desires of the plan are central to everything that we help you with to achieve maximum independence. Together, we identify strengths, capabilities and gifts—and help leverage those gifts to best engage with others and the community.
Comprehensive community services
Residential services (living in Innovative Care Services homes with professional support)
We offer two types of residential services. We have Adult Family Homes (AFH) and Community-Based Residential Facilities (CBRF). When helping select the most appropriate residential supports, our client’s interests, personality, and abilities are all taken into account.
Together, we identify outcomes and the resources needed to meet those outcomes. All of this helps ensure the selection of a home to live with roommates who share similar needs, interests, and desired outcomes.
