Fostering independence through community-supported living
Once you or your loved one has received an evaluation of capabilities and desires, Innovative Care Services develops a specific Individual Support Plan (ISP) with you. From there, we work to ensure the desires of the plan are central to everything that we help you with to achieve maximum independence. Together, we identify strengths, capabilities and gifts—and help leverage those gifts so to best engage with others and the community. Here is a listing of the elements included in everyone’s ISP, and how Innovative Care Services works with you, and all of our clients, uniquely in your own home.
Fostering choice and embracing the dignity of risk
Our clients live in their own homes within our local communities. Clients are encouraged to make their own choices. Our role is to educate and provide options. We support client choice and assist in working together to realize their full potential.
Outcomes-based, not time-based
We don’t live our lives on a rigid schedule and we don’t expect our clients to either. We want our clients to enjoy their independence so we leave once we’ve helped them with the assistance needed that day. Likewise, if something comes up and a client needs support after we leave, we return. The goal is to keep supports flexible and based on what works for each person.
Side-by-side assistance
We view ourselves as facilitators of independence. We want to work ourselves out of a job and the best way to do that is to maximize the independence of our clients.
Community connecting
This supports our clients by building lasting relationships within their community so as to build their natural support base.
Assistive technology
Technology empowers independence for our clients who are living with a disability or aging in place. Assistive technology solutions give individuals the freedom they are capable of and the support they need.
